FAFSA/CADA GRADUATION REQUIREMENT FORM
By answering the questions below you are reporting that you have COMPLETED the Free Application for Federal Student Aid (FAFSA) or California Dream Act Application (CADA) for financial assistance for your incoming college student OR you will be completing and signing the FAFSA/CADA OPT-OUT form with your counselor. 
Important to know:
- By completing the OPT-OUT form with your counselor, it does not waive your opportunity to apply for FAFSA/CADA. If you later change your mind you can still apply for FAFSA/CADA. (This requirement is for High School use only. Does not affect FAFSA/CADA eligibility in the future)
-  If you need support completing the FAFSA/CADA, We will have another FAFSA/CADA Workshop open lab opportunity on Wednesday, February 22 at 6:30pm online via zoom (Click Here for Link). 
- See your counselor to request the OPT-OUT form. 
- Parent acknowledgment required when filling out this form.
