Claims must be received between the start of the purchase period and no later than 30 after the purchase period, after which time no claims will be accepted. Valid for legal residents of the 50 U.S. States, District of Columbia, Canada and Puerto Rico. This offer can not be combined with any other offer or promotion. This claim form must be filled in completely and along with an image of the original UPC code cut from product box, and a copy of the original sales receipt with purchase date, product model name(s), and price paid clearly legible to be valid.
